A Chicago couple is upset after receiving a mailer from OfficeMax last week addressed to Mike Seay ... and "Daughter Killed in Car Crash." Seay and his wife lost their 17-year-old daughter to a car accident last year, and Seay tells NBC Chicago he's wondering, "Why would [OfficeMax] have that type of information? Why would they need that?" The store manager said the situation was "impossible," so NBC reached out; the company is now blaming the incident on a third-party mailing list provider.

"We have reached out to the third-party mailing list provider to research what happened," the corporate office says in a statement. "Based on a preliminary investigation today we believe this to be an inadvertent error; and we are continuing the investigation." But Seay has more questions: "How much other types of other information do they have if they have that on me, or anyone else? And how do they use that, what do they use that for?" He tells the Los Angeles Times he has not received a personal apology from OfficeMax. (More OfficeMax stories.)
