(AP Photo/Brazil's Air Force, file) Today hasn't been the most praiseworthy day for mankind. On the heels of a report that Fukushima was a man-made disaster comes the final report on Air France's 2009 crash, and it cites human error as well. Investigators list a combination of "human and technical factors" as having caused the accident that killed 228, reports the AP. They say the two pilots behind the controls never understood that the plane was in a stall and "were in a situation of near total loss of control." They also cited faulty speed sensors. The BBC notes that 25 safety recommendations were made last year; this report tacks on another 25. (More human error stories.) Report an error
